If you have ever had a testosterone test, you may have noticed that your report can include total testosterone, free testosterone and SHBG. This often leads to a simple question: Which is more important, free testosterone or total testosterone?

Total testosterone is generally the starting point for evaluating testosterone deficiency, while free testosterone can become especially important when total testosterone is borderline or when SHBG may be affecting how much testosterone is available to the body's tissues. Current guidance from the Endocrine Society, American Urological Association and European Association of Urology supports looking at testosterone results together with symptoms and the clinical situation rather than relying on a single number.

What Is Total Testosterone?

Total testosterone is the amount of testosterone circulating in the blood, including testosterone that is bound to proteins as well as the smaller amount that is unbound.

It is usually the first testosterone measurement doctors use when investigating possible testosterone deficiency. The AUA considers a total testosterone level below 300 ng/dL a reasonable cut-off that can support a diagnosis, but the number alone is not enough. Symptoms and repeat testing are important because testosterone naturally changes throughout the day and can also be temporarily affected by illness, poor nutrition, excessive exercise and some medications.


What Is Free Testosterone?

Free testosterone is the portion of testosterone that is not tightly bound to proteins in the blood. Because it is more readily available to tissues, it can provide useful information about androgen status.

However, this does not mean free testosterone is automatically more important than total testosterone.

The AUA does not recommend free testosterone as the primary diagnostic test. It notes that free testosterone can be particularly useful when total testosterone is in a borderline range, roughly 230 to 317 ng/dL, or when a man has significant symptoms despite a low-normal or equivocal total testosterone result.

Free Testosterone vs Total Testosterone: Which One Matters More?

For most men, total testosterone is the starting point. Free testosterone is the additional piece of information that can help explain the picture when total testosterone does not tell the whole story.

One of the reasons is SHBG, or sex hormone-binding globulin. SHBG binds testosterone in the blood and can influence the relationship between total and free testosterone. A man may therefore have a total testosterone result that appears acceptable while having a lower free testosterone level.

This is why the European Association of Urology recommends considering SHBG and free testosterone calculation when conditions that can alter SHBG are present, including obesity, insulin resistance, thyroid disorders, liver disease and certain medications.


Which Testosterone Is More Important for Libido?

Both can be relevant, but testosterone deficiency is more strongly associated with sexual symptoms such as reduced sexual desire than with many of the other symptoms people commonly attribute to low testosterone.

A man with genuine testosterone deficiency may notice fewer sexual thoughts, reduced interest in sex, fewer spontaneous erections or a general decline in sexual desire. However, low libido is not automatically caused by low testosterone. Stress, depression, relationship difficulties, poor sleep, medications, obesity and other hormonal or medical conditions can also reduce sexual desire.

The European Association of Urology notes that sexual symptoms are among the most specific symptoms associated with late-onset testosterone deficiency.

It can.

This is why current Endocrine Society guidance emphasizes that the diagnosis requires compatible symptoms together with consistently low, accurately measured testosterone levels.


What If Total Testosterone Is Normal but Free Testosterone Is Low?

This is one of the situations where free testosterone can be particularly useful.

For example, a man may have symptoms such as low libido or reduced sexual function while his total testosterone is close to the lower end of the reference range. If SHBG is unusually high or low, the total testosterone number may not fully represent the amount of testosterone available to tissues.

In such cases, measuring SHBG and assessing free testosterone can provide additional information. The EAU specifically recommends free testosterone assessment when SHBG is likely to be altered.


What If Total Testosterone Is Low but Free Testosterone Is Normal?

This also needs proper interpretation.

A single low total testosterone result does not automatically establish testosterone deficiency. Testosterone should generally be measured in the morning and, when low, repeated to confirm that the result is consistently low.

The Endocrine Society recommends confirming low testosterone with repeated morning testing, while the AUA uses total testosterone below 300 ng/dL as a reasonable diagnostic cut-off.

The important point is that testosterone deficiency is a clinical diagnosis, not simply a laboratory number.


When Should You Test Free and Total Testosterone?

If testosterone deficiency is suspected, testing is generally performed in the morning because testosterone levels are usually higher at that time. The Endocrine Society recommends confirming a low result with repeat morning testing.

A doctor may consider total testosterone first and then assess free testosterone and SHBG when the total testosterone result is borderline, symptoms do not match the result, or a condition affecting SHBG is suspected.

Other tests such as LH, FSH and prolactin may also be useful depending on the symptoms and testosterone results, particularly when the goal is to understand why testosterone is low rather than simply confirming that it is low.

So, which is more important: free testosterone or total testosterone?

Total testosterone is generally the starting point for assessing testosterone deficiency. Free testosterone becomes especially useful when total testosterone is borderline or when SHBG may be changing the relationship between total and available testosterone.

For libido, erections, strength, energy and quality of life, neither number should be interpreted on its own.

The better approach is to look at symptoms + total testosterone + free testosterone when indicated + SHBG + other relevant health factors.

Most importantly, the goal should not be to find the highest possible testosterone number. The goal is to determine whether a man actually has testosterone deficiency and whether it explains the symptoms he is experiencing.
